Register for Video Consultation.

NOTE: The Chronic Covid Treatment Center is a physician consulting service. All treatment protocols require the involvement of your primary care physician

Once you have received your lab results and have a treating physician, you are ready to register for a video consultation with the Chronic Covid Treatment Center doctors. During the consultation, your labs will be interpreted by our doctors and a treatment plan will be suggested based on our research and algorithms. Please note that we can ONLY provide consultation after you have received your report from the clinical lab and have an MD/DO working with you.

If you are interested in this service, please fill out the registration form completely. Once we receive your registration, we will set you up with your own electronic medical record via simplepractice.com. This platform allows all communication with our doctors, as well as any health information (such as lab results), to be protected by HIPAA. A link to your secure portal will be emailed to you from simplepractice.com within 72 hours after you have registered. Please watch your inbox as well as your spam folder for this email.
