Our Process is Simple.

1 — Register for testing

Fill out the form below to register for a Chronic COVID assessment panel.

2 — Clinical Testing Laboratory will contact you via email to perform your Chronic COVID assessment panel

Our clinical laboratory will send you instructions, and locations of blood draw sites within your area within 48 hours of registering for a test.

3 — Receive your clinical lab results

Access your assessment panel results via a secure lab portal. Due to HIPAA guidelines and patient privacy, please direct all inquiries regarding lab tests and reimbursement to the laboratory.

4 — Connect with Our Clinical Team

After sharing your laboratory results with a primary care physician you may register for a video consult appointment. A link to the registration site for your Telemed appointment will be provided along with your assessment panel results. Register for Video Consultation.

5 — Receive a Treatment Protocol

If warranted, a treatment protocol will be developed and provided to you, and your primary care physician.
